The Mildura Health Private Hospital Oncology Unit provides care to patients requiring Systemic Anti-Cancer Treatment (SACT) and associated medical therapies in conjunction with the Medical Oncologist/s and other practitioners facilitating treatment for specialist haematologists, gastroenterologists, neurologists and rheumatologists.

The role

The Cancer Care Coordinator (0.4FTE) plays a pivotal role in the delivery of comprehensive cancer care. This position involves providing clinical leadership, mentoring team members, and delivering essential care and treatment to patients. A core responsibility is the effective coordination of care across various clinicians, disciplines, and healthcare services to ensure a seamless and patient-centred experience throughout the treatment.

Attend Multidisciplinary Team (MDT) meetings and other relevant forums to support the planning and delivery of patient care

  • Facilitate timely scheduling of appointments, diagnostic tests, and procedures to expedite treatment plans and ensure continuity of care

  • Support seamless care transitions by establishing clear referral pathways and ensuring timely communication between health professionals and across care settings

  • Act as a liaison between patients affected by cancer and members of the healthcare team

  • Undertake comprehensive assessments and screening for clinical, psychological, and supportive care needs at initial presentation and at key points throughout treatment

  • Provide consistent, timely education and information to patients and their families

  • Provide clinical leadership and mentorship within the oncology nursing team

If you would like more hours, up to full time, there is the opportunity to work in an RN capacity on the oncology ward. About you

You are a compassionate and highly organised nursing professional with a strong commitment to delivering patient-centred care. With a deep understanding of the cancer care continuum, you excel at coordinating complex treatment pathways, supporting patients emotionally and clinically, and ensuring seamless communication between multidisciplinary teams.

 You must have:

  • Current AHPRA registration

  • Post Graduate qualifications in oncology (desirable).

  • Previous experience working in oncology setting (desirable)

  • Current National Police Check and Working with Children Check

  • Up-to-date required immunisations

  • A caring, empathetic approach to patient care
